jean-pierre charras f25d74628b Plot pad outlines on Fab files: fix many issues. Depending on 
plotter type and pad type, the plotted shapes were not always the outlines,
but a outline inside the pad shape (due to legacy reasons). In many cases the
plotted shape was in fact incorrect. Now the actual pads outlines are plotted
without modification.

jean-pierre charras e14e9cf8a9 PLOTTER: do not clamp coordinates to an arbitrary value. 
Previously, coordinates were clamped to +- 60 inches. It makes no sense to
clamp them at plotter level: max coord depends on the editor (schematic/board...)

Jeff Young b52529521e Replace individual LIB_* shapes with LIB_SHAPE (based on EDA_SHAPE). 
Also moves to more capable FILL_T model that can be shared.

Seth Hillbrand 52bbfb9109 Remove dependency of dot size on linear mils 
Changes a dot to be a square pixel (linewidth x linewidth).  This allows
the removal of IU dependencies and ensures that a dot is always visible
on screen.  Also makes sure that cairo is setting the current linewidth
during its stroke routines

Ian McInerney ecd9bf696e Be consistent about using nullptr instead of 0 
0 can be too easily overloaded into an int, so ensure we use nullptr
when we mean pointers.

jean-pierre charras 380534807b Plotters: better support of filled circles having a thickness set to 0. 
Setting a line thickness = 0 for graphic objects (circle, rect, line) is
allowed but creates issues for circles that were plotted a non filled shapes
but having a outline thickness = 0.

Jeff Young c7da63151c Clamp egregiously large numbers when plotting. 
This keeps some plot file viewers (such as Acrobat) from choking.

Seth Hillbrand e38ab6c699 Treat 0-length ovals as circles 
Some buggy SVG interpreters can't handle 0-length ovals.  In this case,
they are meant to be circles, so we can export as circles in our
plotter.

Seth Hillbrand c6f5df134c Minor speed cleanup 
This adjusts iterators to use const reference when only used for
copy.  It also ensures pre-allocation of vectors when size is known
ahead of time.

Seth Hillbrand 53989e3bdc SVG: Correct plot fills for arcs/polylines 
When plotting in Eeschema, various elements may be filled with either
the foreground or background colors.  The fill mode is rather unique,
requiring un-stroked pie wedges for arcs and both closed and open filled
polylines.
