Push most of DIALOG_CHOOSE_SYMBOL down into PANEL_SYMBOL_CHOOSER.

Includes a dialog wrapper (DIALOG_SYMBOL_CHOOSER) and a frame
wrapper (SYMBOL_CHOOSER_FRAME).
