So the new steals-focus protection stuff works well when there's a
focused control, but not as well when there's a modal dialog up which
happens not to have a focused control (or worse, a focusable control).
This adds a second mechanism for also checking to see if a modal dialog
is up (something that wxWidgets, true to form, makes very difficult).

The infobar changes the size of the visible screen, so adding/removing
it will shift the screen center, causing the position of onscreen
objects to move up/down. This changes the sizing to pin the bottom
corner position when showing/hiding the infobar

For instance, we get things like high-contrast-mode from the pad
dialog when rendering a pad preview, but we want the colours from the
parent PCB_BASE_FRAME.
On platforms with limited framebuffer attachments, like GC7000L
driven by the open source etnaviv driver, we can still use
GPU acceleration by drawing directly to the main buffer.
EDA_DRAW_PANEL_GAL::onPaint checks if an overlay target is
available in the GAL, and if not, redraws the whole view if the
overlay target should be dirty.
Clearing of the overlay target is a no-op if there is no overlay
buffer.

The canvas size is not guaranteed to be strictly > 0, so we need to
enforce a minimum returned size before dividing by it or passing to GAL
initialization

Settings are now managed through a central SETTINGS_MANAGER held by PGM_BASE.

Per-application settings are now stored in one class for each application.
The thumb size changes between system versions and (on GTK) render
managers, so hard-coded wasn't working. This gets the actual value from
the system and uses to adjust the range. Changes in the range can also
trigger the refresh.

Includes the addition of an onSetCursor() handler which must be called
from both the GAL canvas AND the GAL backend (at least on OSX) to prevent
cursor flickering between (for instance) pencil and arrow.
Also includes new architecture for point editors which allows them to
coordiate cursors with the editing tools (so we can switch to an arrow
when over a point).
GTK3 does a fade in/fade out routine for scrollbars by default. This
was problematic as it caused a cascade of full-screen repaint events on
each mouse event. Instead of disabling scrollbars (which only helped
the full canvas), we force the old scrollbar behavior is environmental
variables.

The canvas type might change without the frame being notified. If this
happens, the user is presented with repeated notifications of OpenGL
issues. Howver, switching canvas is not an error and should be
presented to the user as an info message to clarify that the system is
still working but it has changed how it renders the screen.

Under GTK3, the scrollbars fade in and out, causing a cascade of
re-paint events. These cause KiCad to be unresponsive while the repaint
events occur. By default this is 1s fade in 1s fade out but can be
configured by the user/wm.
Area mapping the repaint events to just scrollbar areas will be a good
solution but until/unless that happens the scrollbars in GTK3 are
problematic.
When the error switching to OpenGL is triggered by the requested switch,
we want to fallback to the cairo GAL renderer instead of the
GAL_TYPE_NONE, which defaults to legacy.

Display GALs had an incorrect world unit value set. Now the world unit
value says how many internal units are in an inch, in accord with the comments
in the GAL header. Bitmap drawing code relied on the information about DPI,
so scaling worked differently for display and print GALs.
Fixes a crash when typing fast in the place footprint filter box.
Also adds a bunch more checking to GAL locking, including making
sure the same person unlocks as locked, and preventing piece-meal
calls (the RAII objects must be used).
This prevents deadlocks when exceptions are thrown and the context
ends up not getting unlocked.
It also removes an earlier hack to try and minimize this which
didn't work anyway.

3) make sure diff pair radius is always 100%

This puts the graphical/display options with the other similar options
in that dialog, rather than with the dimension options found in the Set
Grid dialog, which is in the Dimensions menu.
Also place the option itself in the GAL Display Options structure, so it
can be updated using the observer mechanism there.
The setter for the style in the GAL interface is removed, as the public
interface for setting this option is now to modify the
GAL_DISPLAY_OPTIONS structure and notify the GAL when done.
Some faults could result in a crash, as they were not properly
handled. Now the rendering loop is wrapped with try..catch block
which will revert to Cairo in case of an error and display an
error message.

Add a hack so that the pad properties base dialog constructor can pass in the dependency. Since the derived dialog's constructor takes the parent by this exact type, the static cast is safe.
